This is not just another questionnaire. Lewisham LINk believes that carers are real experts and should be involved in how care needs are decided and how care is delivered, with the person's permission if they can give it.
We want to find out if you are involved when decisions are made and use this information to Improve Carers involvement with service user assessments and support planning.
Carers Matter
A Carer is someone of any age who provides unpaid support to family or friends who could not manage without this help due to illness, disability, mental ill-health or substance misuse problem.
Carers are a hidden workforce. Many have incredible reserves of commitment, passion, knowledge and expertise. They are often the experts in the care of their parent, son, daughter, sibling or friend and have knowledge of their journey through life - the history that informs their identity and shapes their future. Offering good support to people with illness, disability, mental ill-health or substance misuse problem means offering good support to the people closest to them.
